At the admission time of hospitalization, be certain of holding the following documents:
- All medical records, consisting of images, examinations, etc.
- patient’s passport
- physician hospitalization order form
Written confirmation must be taken for the surgeries mentioned below:
- Cosmetic surgeries
- Surgeries relating to accidents, conflicts, suicides
- abdominal hernia, types of abdominal surgeries (abdominoplasty and liposuction and its complications) and breast (mammoplasty and its complications) that have a cosmetic aspect
- types of surgeries associated with weight loss
- strabismus, maxillofacial surgery, and Hallux Valgus
- septoplasty
- vasectomy, TL, Vaso-vasoplasty and infertility surgeries
- uterine curettage
- any type of reconstruction (correction) surgery for old scars anywhere on the body
- maxillofacial surgery
In hospitalization for ACL (Anterior cruciate ligament), scoliosis, and kyphosis surgeries, the original radiographic and pre-operative MRI results are needed before admission, which must be delivered to the hospital.
The ultrasound or CT scan results before the operation are required in TUL (Transurethral Lithotripsy) and PCNL (percutaneous nephrolithotripsy) surgeries.
Notice that not holding these items with you will cause the lack of hospitalization and cancelation of your surgery.
Please note the following hints before your beloved child’s operation and be sure to have the following documents with you:
- Have images (radiographs), examinations, CT scans, etc. with you.
- The attendance of the child’s father or legal guardian with a validpassport at the time of admission is imperative.
- Attendance of the father for signing the documents is essential only by an identity card if the patient is under 18 years of age.
Notice that not holding these items with you will cause the lack of hospitalization and cancelation of your surgery.
